The Software is part of the system, but it is also more prone for human errors and as such, more critical for functioning of the Complex System. There is another thing which makes the Software very critical for Complex System’s operations. It has been shown that most of the Software-related faults and failures in Complex Systems are caused by the flaws in the requirements specifica- tions or they were not clearly determined. The point is: If there is a wrong requirement about specification, it cannot be detected, because it will pass every model, simulation and experiment for checking the Complex System. There are many methods and methodologies which can be used to provide Risk Assessment of Software. I would not go into details and I will not propose any of them. Just I would mention most used (as per literature on Internet). The most used methods are Software FMEA, PHA, Software FTA, and they (applied together) can be found under one name as HAZAPS (HAZard Assessment in Programmable Systems) methodology. However, I do not think and do not support approach that the Risk Assessment of the Software should be done by ordinary Safety Professionals.
This book will appeal to any safety professional tasked with complex systems. This extends to professionals in risky industries such as aviation, nuclear power, chemicals, railway and transport, and pharmaceuticals.
